The following is a review of the literature on use of the omentum as a vascularised transplant. We describe a single case from our own clinical use of the omentum as a "buffer" layer between transplanted muscle and recipient area in the forearm.

The interpositional layer of the omentum prevents the process of cicatrical adhesion between the muscle and the tendon and reduces tissue resistance at sliding that immediately results in improvement of function. The application of plentiful vascular network of the omentum increases the muscle trophic and thus, improves the functional result.

Analyzing our experience, we put indications to the multi-purpose transplantations including simultaneously muscle and the omentum as a method of one-stage reconstruction of the upper extremity.
